Pre & Post Care in Hair Transplantation : A Guide for Achieving the Best Results

A hair transplant is not just a cosmetic procedure; it’s a medical treatment. The success of a hair transplant begins even before the procedure. To make the procedure a successful and get natural outcome, proper plan before the procedure and care after the procedure is critical. It helps in achieving the best result.

Whether one is planning for his first hair transplant or expecting a good looking result after the procedure, a good understanding of pre-care and post-care is essential. It makes a big difference in recovery, healing and final appearance of hair.

Before we dive deep into what to do before and after a hair transplant, following are few real case studies, data, and expert-backed insights that reflect how pre and post care influences the result.

Findings in Research on Pre and Post Care:

  1. In a 2024 Clinical Study conducted by the Indian Journal of Dermatologic Surgery, it was found that by following proper post-care instructions the graft survival rate can increase up to 98%, while poor hygiene or early scratching can lead to 10–15% graft loss.
  2. Journal of Cosmetic Dermatology studies (2022) report that patients who initiate gentle washing after 72 hours experience faster scab clearance (by 30%) and reduces infection risk as compared with those who did washing after a week.
  3. ISHRS also conducted a study on the impact on pre- and post-care and published it in its Global Practice Census in 2023. That states an average 85–95% of grafts transplanted are successful grown when proper post- and pre-care is followed.

The importance of pre and post procedure care on achieving good result can be more clearly understood through our case study. While doing our studies we came across following findings.

Case Study 1:

The first study was done on a 32 year-old patient names Ravi, a tech professional from Delhi, who was a smoker for 10 years. He followed the advice from the doctor to abstain from smoking for at least 10 days before the procedure day and continue the same till 4 weeks after the hair transplant. He did that religiously. When assessment was performed after hair growth, it was found that 95% of the grafts survived, compared to an average of 80–85% in smokers who did not quit before the surgery.

Learning: Discontinuing smoking and staying healthy improves blood circulation, results in high graft survival.

Case Study 2:

One another case was of a 41-year-old patient from Mumbai. He did not follow elevation instructions during sleep, developed forehead swelling lasting 7 days. Though no damage to grafts occurred, recovery was delayed by nearly a week.

Learning: Small post-care steps like proper sleeping posture prevent complications and speed up recovery.

Case Study 3:

A patient who included protein shakes and biotin-rich foods post-surgery noticed visible regrowth at 3 months compared to the average 4–5 months’ timeline.

Learning: A nutritional diet accelerates recovery and hair growth in patients. So, one should eat nutritious food.

What are the Before (Pre) Care and After (Post) Care

From the above discussion, it is well established that the outcome of the procedure very much depends on how well one prepares before the surgery and how much one follows the aftercare instructions. Care at both phases plays an equal and important role in ensuring graft survival, healthy regrowth, ensuring a smooth and complication-free recovery and achieving natural result. The following are important pre- and post-care points that one should diligently consider.

Before the Hair Transplant: Pre-Care Guidelines

Proper preparation of the mind, body, and scalp ensures not only smooth procedure but also faster healing, proper graft growth and provided desired outcome. Following are the guidelines for pre procedure consultation and care:

1. Consultation and Medical Check-up
  • Every patient must undergo a detailed consultation with the doctor for analysing the scalp condition, donor area strength and hair density, and must discuss medical history, on-going medications, and allergies he has.
  • If all goes well, the doctor ask patient to undergo a blood test or scalp analysis to evaluate any medical issues.
  • One must go through a pre-procedure medical test (surgeon usually gives a list of test at the time of consultation) that includes CBC, PT/INR, viral markers (HIV, HBsAg, HCV), Blood sugar(diabetes), Blood Group, ECG (if required) ,thyroid profile, etc. Without normal report of these test s, clinic cannot do a procedure.(Note: A patient should follow the test list provided by his consulting doctor).

2. Avoid Blood Thinners, Alcohol & Smoking
  • It is advised that the candidate, the patient should stop taking aspirin, ibuprofen, vitamin E or fish oil, and any blood-thinning medications at least 7–10 days before the surgery, (take only after doctor’s approval).
  • Don’t consume alcohol at least for a week prior, as it reduce blood flow and slow down healing. Alcohols dilate blood vessels and increase the risk of bleeding.
  • Similar to alcohol, smoking also reduce blood flow, nicotine constricts blood vessels, reduces oxygen in blood and slow down healing. It should also be stopped a week prior to procedure. Quitting smoking increases oxygen flow in blood and enhances post procedure healing process.

3. Hair & Scalp Preparation
  • The patient should wash their hair one day before the surgery using a mild shampoo (baby shampoo) or doctor-recommended shampoo.
  • If one is suffering from dandruff or scalp infection, or psoriasis he must inform the doctor well in advance.
  • If patient lies in case of those who is suffering from dandruff or having scalp infection, patient should get the treatment before planning the procedure. It is recommended that the patient should have a healthy scalp at the time of the procedure.
  • It is strictly advised that the patient should abstain from applying any oil, styling gel, or spray on the day before the procedure after the hair wash or on the day of procedure.

4. Clothing & Comfort
  • On the day of the procedure patient should wear front-open (loose button-up) shirt or a zippered jacket. The idea is whatever a patient wore can be removed completely without touching or disturbing the head. Such clothing ensures no disturbance to the newly transplanted grafts while changing the clothes.

5. Foods & Breakfast
  • Eat light breakfast (unless instructed otherwise) on the day of procedure.
  • Food having caffeine should not be consumed in any form either by eating or drinking. Caffeine may increase bleeding during the procedure and even after.

6. Travel Planning
  • Arrange transportation post-surgery and one of patient’s friends or relative or family member should be accompany him during pre-planned travel, procedure and stay. It is advised because many experience drowsiness due to mild sedation or local anaesthesia after effect.

After the Hair Transplant: Post-procedure Care Guidelines

By following the post-procedure care, the patient not only ensures that his recovery will be infection-free, swelling minimize timely, and but also a high success rate of graft survival is assured. Pre- procedure discussed earlier is just a one step in right direction for achieving expected result. After the transplants follow the instructions given by the doctors carefully, it can be understood in following points:

1. Travelling back to home & place of stay
  • Since the procedure is single day process, arrange someone to company the patient back to home, as patient may feel drowsy due to local anesthesia or sedation.
  • The patient should arrange his stay in clean and thoroughly disinfect space as he will stay there for the entire recovery process. He should use clean pillows and soft towels.

2. Rest & Recovery (First 48–72 Hours)
  • Usually a complete rest is advised but it does not mean the bed rest. One can meet his daily activity by his own but should not take strenuous activities.
  • Avoid bending and heavy lifting.
  • While sleeping one should keep his head elevated at 45 degrees for 3–5 nights that helps in reduce the swelling.
  • Patient should not scratch, or rub and also should not repeatedly touch the transplanted area to safe the newly placed grafts from being disturbed.

3. Washing & Cleaning
  • Doctor usually allows the first gentle wash after 48–72 hours.
  • Avoid direct pressure from showers; and pour water gently using a mug or cup instead of direct pressure.
  • Use only the recommended mild shampoo (baby shampoo).
  • Avoid use of hot water for washing head.
  • Don’t use towel for drying, let hair dry naturally.
  • Do not pick scabs; they naturally fall within 10–12 days.

4. Medication & Healing

One may experience emergence of mild swelling around the forehead may within 24 hours. No need to worry or panic if this transpires. This is normal, usually resolves within 3-4 days.

  • The prescribed medicines should be available and should be kept in reach of the patient.
  • Timely take antibiotics, anti-inflammatory medications and pain relievers as prescribed.
  • One should avoid self-medication during this phase; it could negatively impact the grafts survival and growth.
  • Apply any topical lotion or spray only if recommended by doctor.
  • One may notice mild scabbing, redness, or itching, this is normal and part of healing.
  • If one is taking any other medicine due to a previous medical condition that the hair transplant doctor does not prescribe, one should consult his previous doctor and also discuss about the medicines with the hair transplant doctor at the time of consultation.

5. Avoid Direct Sunlight, Sweat, workout and Dust
  • Avoid direct exposure to sunlight as it can damage new grafts. It should be followed at least for 2–3 weeks.
  • Avoid heavy works that cause sweating around the newly transplanted grafts to prevent infection. Don’t do gym workouts, weightlifting, swimming, running, steam bath, and dry or wet heat sessions in saunas for 2–4 weeks.
  • If necessary, cover head gently with a cap to avoid dust and reduce the risk of infection.
  • Don’t wear helmet for 3-4 weeks.
  • If someone’s profession require wearing helmet, please avoided it, instead one should take the permission from his authorities at work. Cap can be worn that too with gentle hands for the first few weeks.

6. Hair Care & Maintenance
  • During healing period avoid using hair oils, gels, or sprays for 4-5 weeks or until patients hair transplant doctor allows.
  • After 3–4 weeks, one can return to your normal hair care routine.

7. Lifestyle
  • As advised in pre procedure cares, avoid smoking further for at least 4 weeks after hair transplant. It enhances success rate by maintain the vital oxygen supply and nutrients, reduces infection risk and boosts graft survival.
  • Don’t wear tight-fitting caps or helmets to avoid direct pressure or friction on the transplanted graft.
  • At this stage it is advised to avoid the use of harsh shampoo in the first 4 weeks.
  • Sleep well for 7–8 hours daily to supports tissue repair and graft nourishment.

8. Food & Nutrition

A balanced diet and stress management greatly influence transplanted graft’s health. Following points on food and nutrition should be considered while planning post procedure diet.

  • It is advised to include protein-rich foods like paneer (cheese), egg, fish, lean meat, lentils and nuts.
  • Include green leafy vegetables like spinach to get overall nutrition in daily diet.
  • Include vitamins reach foods especially that contains B7 (Biotin), Vitamin D and E for stronger regrowth.
  • Source of minerals like Zinc and Iron should also be included in diet. They are very important for hair strength and regrowth.
  • It is very important to stay hydrated, drink at least 2-3 litres of water daily.

9. Don’t miss post-procedure follow-ups.
  • Patient should stay consistent with follow-ups and scalp care routines recommended by the doctor.
